Services

Heritage Family Health is a full-scope family practice. Below is the care we plan to offer by appointment only when we open in Fall 2026. Do you already love your primary care doctor? We welcome visits for consults or specialty services only.

We believe in clear, transparent pricing for healthcare. Prices below are cash pay for physician or midwifery services and due at the time of service. Itemized receipts are available to submit for insurance reimbursement, and we accept FSA/HSA cards. Lab and imaging tests and medications are billed separately. We will add a list of insurances accepted as they become available. Pacientes hispanohablantes son bienvenidos.

Heritage Family Health offers a sliding‑scale fee program to ensure access to care for all patients, regardless of income or insurance status. Discounts are based on household income and family size. Please contact us with questions about cost.

Note: traditional Medicare pays for this visit only after one full year has passed since your last one. Medicare replacement plans may allow one visit each calendar year.
