The only information this website collects is what you choose to submit through our notification form: your name and email address. We use it for a single purpose — to tell you when the practice opens and to answer questions you send us.
- We do not sell, rent, or trade your information.
- We do not share it with advertisers or data brokers.
- Information is stored with our hosting and database providers, who are bound by their own security obligations.
- You may ask us to delete your information at any time by emailing info@heritagefamilyhealth.net.
Please do not send medical details, test results, or other protected health information through this website. Email and web forms are not secure channels for health information.

Once Heritage Family Health begins providing care, we are required by law to maintain the privacy of your protected health information, to give you notice of our legal duties and privacy practices, and to follow the terms of the notice in effect.
